How do I log into the Prescription for Play portal?
To log into the Prescription for Play portal, you must have first created an account. If you have not already done so, click here for instructions or here to do so. Please do not create an account unless you meet the Eligibility Requirements of the program.
- Once you have created your account, you can log in either by:
- Navigating in your browser to https://www.rx4play.org/login/index.php, or
- Clicking the Log In link at the top right of the home page.
The Log In page will display. - Enter the username and password you configured when creating your account. Then click Log In.
- If you have forgotten your password, click the Forgotten your username or password? link.
